1/1/2023 EnchantaVaughn Approved Behaviors: Behavior: Upon arrival, client was engaging in conversation with mother while his siblings were playing a game. Client appeared to be in a happy mood, as evidenced by client smiling during conversation with mother. Client was dressed in decent attire and appeared to have came home from school in a good mood. Client spoke to his counselor about his day and what he looked forward too going into the week. Client displayed appropriate behaviors while reviewing today’s lesson. Behavior: Client presented himself to be satisfied with the provided activity. Client dislayed good conduct and was cooperative throughout the session. Client displayed good level of participation, completing psycho educational, and making good choices. Client was able to demonstrate a good attitude and work independently. Client was proud that he could offer a more positive environment for his family. Intervention: During conversation at the home, QMHP communicated with client and mother about client’s progression and possibilities of goals and objectives during continuation of services. QMHP encouraged mother to point out possible stressors and/or irritations that may trigger client by behaving negatively. QMHP continued to engage with client and provided positive reinforcement as he participated in the session. Intervention: QMHP focused on increasing structure in the home to aid client in acknowledging the importance of operating and cooperating in the home setting. QMHP applied talk therapy and active listening skills at this time. QMHP commended client on utilizing creative strategies to increase his understanding of feelings and emotions. QMHP commended client for his participation and honesty. Response: During the session at the home, mother expressed client’s behaviors have improved and are more consistent. Client smiled as he was affirmed for his mature and pleasant behaviors. Client informed QMHP that he was coping and utilizing deep breathing techniques to stay calm and not overreact. Client was receptive and open to the interventions, as he was attentive and focus on the information and activities that were being processed. Response: It was noted that the interventions were successful. Client was able to relax and regain and regulate his overactive behaviors throughout the session. Client was receptive and open, extending a healthy rapport back to QMHP. Client communicated that he felt safe and comfortable talking to QMHP. Client exhibited great manners at this time. Client acknowledged QMHP’s support as session concluded.
